In 2018, Catholic Charities teamed up with St. Mary’s Hospital to develop the Health Connect Program. The program helps patients that utilize the emergency room 6 or more times in a 6-month period. The hospital will refer patients to the Community Health Worker (Jennifer Reynolds).

The program assists with:

• Helps patients gain a better understanding of their health.
• Helps patients find primary care physicians.
• Helps transport patients to their appointments.
• Helps patients find resources in the community such as food and housing.
